Dr Manish Tiwari Lip and Palate Cancer Surgeon

Lip Ulcers / Crusting

A persistent sore, lump, or thickened area on the lip that bleeds or won't heal.

Palatal Lumps

A painless or painful swelling on the roof of the mouth (hard or soft palate).

Nasal Speech

A change in voice resonance or difficulty speaking clearly due to soft palate issues.

Dentures Not Fitting

Upper dentures suddenly feeling loose or uncomfortable due to a mass on the palate.

Types of Excision & Reconstruction

Cancers in these areas require extreme precision. Dr. Tiwari utilizes advanced oncological protocols to remove the tumor while utilizing plastic surgery techniques to restore function.

Lip Resection & Reconstruction

Excision of the lip tumor (Wedge excision). The defect is immediately reconstructed using local advancement flaps (e.g., Karapandzic or Abbe-Estlander flaps) to maintain the shape and function of the mouth.

  • Focus on preserving the oral sphincter
  • Restores lip competence to prevent drooling
  • Highly aesthetic cosmetic closure

Hard Palate (Maxillectomy)

Tumors on the roof of the mouth often invade the upper jaw bone. A partial or total Maxillectomy removes the diseased bone to ensure cancer eradication.

  • Removes deep-seated bony tumors
  • Immediate Free Flap or Obturator placement
  • Prevents food passing into the nasal cavity

Soft Palate Resection

The soft palate controls speech resonance and swallowing. Tumors here require delicate resection followed by complex soft-tissue reconstruction to restore the velopharyngeal mechanism.

  • Radial Forearm (RAFF) soft tissue flaps
  • Preserves swallowing capability
  • Prevents severe speech impairment
The Protocol

The Surgical Pathway

A highly structured approach ensuring no cancer cells are left behind, paired with world-class reconstructive techniques.

01

Primary Tumor Excision

Meticulous surgical removal of the tumor with a 1cm to 1.5cm safety margin around the lip, or bony margins along the palate.

02

Neck Node Dissection

Advanced cancers of the lip or palate can spread to the neck. Lymph nodes are selectively cleared to prevent systemic metastasis.

03

Defect Reconstruction

Using local tissue flaps for the lip, or sophisticated Microvascular Free Flaps (like the Radial Forearm Flap) to seamlessly rebuild the palate and close nasal communications.

Diagnosis & Recovery

Diagnostic Protocol

Understanding the depth of the tumor is crucial for planning the resection.

  • Incisional Biopsy: Confirms the presence of squamous cell carcinoma.
  • CT Scan (Face/Neck): Evaluates bone invasion in the hard palate.
  • MRI: Maps out soft tissue involvement in the lip and soft palate.

Post-Operative Rehabilitation

We ensure you regain the ability to smile, speak clearly, and eat comfortably.

  • Prosthetics (Obturators): Custom dental plates to seal the palate if flaps aren't used.
  • Speech Therapy: Assisting in vocal resonance recovery.
  • Cosmetic Healing: Advanced suturing techniques to minimize lip scarring.
